WEbRx Lifesystems Vaccine Policy

WEbRx Lifesystems is committed to providing a safe and healthy workplace for all our employees, customers, and visitors. As part of this commitment, we have developed this vaccine policy to protect our community from the COVID-19 pandemic and to comply with the local, state, and federal laws and regulations.


Scope


This policy applies to all employees, contractors, interns, volunteers, and visitors who work at or visit our company premises, or who interact with our customers or vendors on behalf of our company.


Policy Statement


WEbRx Lifesystems strongly encourages all eligible employees and visitors to get fully vaccinated against COVID-19 as soon as possible unless they have a valid medical or religious exemption. Vaccination is one of the most effective ways to prevent the spread and severity of COVID-19 and to protect yourself and others from the virus.

WEbRx Lifesystems requires all employees and visitors to provide proof of their vaccination status before entering our WEbRx premises, or before engaging in any work-related activities that involve contact with our customers or vendors. Proof of vaccination status can be provided by showing one of the following documents:

A vaccination card or certificate that shows the name, date, and type of vaccine received.

A digital record or app that verifies the vaccination status.

A letter or statement from a health care provider or a public health authority that confirms the vaccination status.

WEbRx Lifesystems will keep a confidential record of the vaccination status of all employees and visitors and will only use this information for the purpose of implementing and enforcing this policy. Our company will respect the privacy and confidentiality of all personal information and will comply with the applicable laws and regulations, such as the Health Insurance Portability and Accountability Act (HIPAA).

Our company will provide reasonable accommodations for employees and visitors who cannot or do not want to get vaccinated due to medical or religious reasons, unless doing so would pose an undue hardship or a direct threat to our company or others. Employees and visitors who request an accommodation must provide a written documentation from a licensed health care provider or a religious leader that explains the reason for the exemption. Employees and visitors who are exempt from the vaccination requirement must comply with the following alternative measures:

Always wear a face mask while on WEbRx premises, or while interacting with our customers or vendors.

Maintain a physical distance of at least 6 feet from others.

Undergo regular COVID-19 testing at least once a week, or as directed by WEbRx.

Follow all other health and safety protocols and guidelines issued by WEbRx and the public health authorities.

Our company will cover the cost of the COVID-19 vaccines and testing for all employees and will reimburse the expenses incurred by employees who get vaccinated or tested outside of our company. Our company will also provide paid time off for employees who need to get vaccinated or recover from any side effects of the vaccine.

WEbRx Lifesystems will communicate and educate our employees and visitors about the benefits and safety of the COVID-19 vaccines and will provide accurate and updated information and resources on the vaccines. Our company will also solicit feedback and input from our employees and visitors on this policy and will address any questions or concerns that they may have.

Our company will monitor and evaluate the effectiveness and impact of this policy, and will make adjustments as needed based on the changing circumstances and feedback. Our company will notify our employees and visitors of any changes or updates to this policy in a timely manner.


Compliance and Enforcement


WEbRx Lifesystems expects all employees and visitors to comply with this policy and to cooperate with our company in implementing and enforcing this policy. WEbRx reserves the right to deny entry or access to WEbRx premises, or to terminate or suspend the employment or contract of any employee or visitor who fails to comply with this policy, or who provides false or misleading information about their vaccination status. Our company will take appropriate disciplinary actions against any employee or visitor who violates this policy, in accordance with our company policies and procedures, and the applicable laws and regulations.
